The NCVIA also requires that certain things be included in the patient’s chart.

A. A parent’s signature acknowledging receipt of the VIS sheet or that it has been explained to them

B. The date of the last revision of the VIS sheet

C. The date the vaccine recipient received the VIS sheet 

D. The name, address, and title of the person who administered the vaccine

Many physician offices still routinely get a parent’s signature for consent for medical malpractice protection, but it is not required by federal law. Some states require a signature. Other things that need to be recorded per federal law include the vaccine manufacturer and lot number of the vaccine used.
