You have just given 55-year-old Sam Smith the news that he has type 2 diabetes. Mr. Smith has no history of cardiovascular disease or other comorbidities.

His HbA1c is 9.1; his BMI is 35; triglyceride level, 220 mg/dL; HDL 37 mg/dL; blood pressure, 150/94 mm Hg. His renal function is normal.
